Our Strategy

Overview

Queen Square’s 2024–2027 Strategic Plan reflects our continued growth and commitment to delivering equitable, high-quality care. Informed by Ontario Health’s Equity, Inclusion, Diversity and Anti-Racism Framework, the plan prioritizes accessible services, inclusive environments, and meaningful progress for both patients and providers. Our Strategic Plan

In 2023, Queen Square Doctors & Family Health Team began the process of renewing our joint strategic plan with the purpose of optimizing the care experience for both our patients and our providers. It also allowed us to reflect on the progress we have made since the implementation of our previous strategic plan, which came amidst the early stages of the COVID-19 pandemic. Since then, our team and impact within the community has grown significantly, and this growth is reflected in the renewal of our Strategic Plan for 2024-2027. Through planning and action, each member of our team is committed to achieving the goals set out in this plan and continuing to expand our ability to provide equitable and high-quality care to our patients and community.

 

In determining the new strategic direction of Queen Square for 2024-2027, a detailed and action-orientated plan was devised by staff to optimize health care resources and enhance experiences. 


Key areas of focus and priorities include:

We are committed to providing patients with equitable, timely and high-quality care.


  1. Collaborative Culture

We will promote patient-centered collaboration across our team, within our community, and in the education of the next generation of healthcare professionals.


  1. Enhanced Team & Provider Experience

We will invest in the quality improvement of our services, and the wellness and professional

development of our team.


  1. Safe Spaces

We will ensure that through the principles of Equity, Diversity, and Inclusion, our organization and practice is a space where all people are welcome and can strive towards reaching their fullest health potential.


  1. Information & Technology

We will leverage effective communication and technology to optimize patient access to services and to create more efficient workflows.

Equity, Diversion & Inclusion

The values of Equity, Diversity and Inclusion were used to inform the creation of this plan and will be used to inform the implementation of this plan. Queen Square is committed to the 11 areas of action outlined in Ontario Health’s Equity, Inclusion, Diversity and Anti-Racism Framework. This will support our vision of improved health outcomes for patients and families and build superior work environments for our health care providers. 



Ontario Health's Equity, Inclusion, Diversity, and Anti-Racism Framework outlines 11 key actions, such as embedding strategies, engaging key voices, addressing racism, and promoting Indigenous health equity, to create a fair and inclusive healthcare system.



Equity 

Equity refers to the absence of unjust differences between groups and individuals such as sex, gender, ethnicity, disability and sexual orientation. Health equity is achieved when individuals have the opportunity to attain their fullest health potential. We can enable this by acknowledging, reducing and removing barriers to care and providing individuals with the services that best support their needs.

Diversity  

Diversity refers to the variety of visible and invisible qualities, experiences and identities that we possess. Some dimensions of diversity include race, ethnicity, gender identity, sexual orientation, socio-economic status, language, age, physical or mental abilities, lived experiences, and religious, spiritual, and political beliefs. Diversity can be achieved by including individuals from a variety of backgrounds to represent their experiences and inform new ideas.

Inclusion 

Recognizing diversity and ensuring that all individuals feel welcomed, respected, and accepted is known as inclusion. Inclusion is being actively mindful of the unique experiences that individuals bring to the table and involving them in decision making processes. Inclusion is promoted by welcoming diverse lived experiences, skills, and qualities to enable individuals to thrive while being their true selves.

Queen Square Family Health Team operates on the Treaty Lands and Territory of the Mississaugas of the Credit First Nation. We acknowledge the territory of the Anishinabek, Huron-Wendat, Haudenosaunee and Ojibway/Chippewa peoples; the land that is home to the Metisl and most recently, the territory of the Mississaugas of the Credit First Nation who are direct descendants of the Mississaugas of the Credit. We are grateful to have the opportunity to work with and serve the people who inhabit this land, and by doing so, give our respect to its first inhabitants.
