This holistic approach supports people of any age by improving their quality of life through the management of symptoms like pain, nausea, and anxiety. Our interdisciplinary team works with patients and their families to provide emotional, psychological, and spiritual support, ensuring care is aligned with each patient's personal goals and wishes.

What is Palliative Care?
Palliative care is a holistic approach that supports people of any age with serious illnesses, in any setting. It focuses on improving quality of life by managing symptoms such as pain, nausea, anxiety, and difficulty breathing while also providing emotional, psychological, social, and spiritual support. This type of care can be provided alongside treatments aimed at curing an illness, ensuring that both the patient and their caregivers feel as comfortable and supported as possible.
Who is this for?
This program is for patients (18+) who are rostered with a family physician at Queen Square Doctors with a serious or life-limiting illness.

Our interdisciplinary team collaborates to provide holistic, patient-centered support that prioritizes dignity, comfort, and quality of life. We proactively address distressing symptoms while guiding patients and families through end-of-life transitions in their choice of setting. Our team works to ensure treatment aligns with patient values, easing suffering and supporting both patients and their loved ones.

This program is delivered by a collaborative team that, based on your needs, may include:

What services are included?
Management of symptoms such as pain, nausea, anxiety, and difficulty breathing.
Emotional, psychological, social, and spiritual support.
Advance care planning to ensure treatment aligns with patient values.
Grief and bereavement support for patients and their loved ones.
Ongoing assistance for patients and caregivers from diagnosis through all stages of illness.

Please speak with your family doctor or nurse practitioner to determine if palliative care is the right choice for you and to initiate a referral to the program.
Our Post-Hospital Discharge Follow-Up Program
Transitioning from the hospital to home can be challenging. Our Post-Hospital Discharge Follow-up program provides comprehensive support for a safe and smooth recovery. Our collaborative team of nurses and providers works with your family doctor to review discharge instructions, coordinate follow-up care, and answer your questions. Our primary goal is to prevent complications and readmissions through clear, timely support, creating a crucial bridge of care during your recovery.
