Community Partner Physicians of Queen Square Family Health Team are taking new patients
In alignment with Ontario Health’s initiative to attach patients to family physicians, we are partnering with community physicians to provide family doctors for patients who do not currently have one.

Why are we expanding?
Our expansion is patient-driven. We proactively approached Ontario Health to request funding because we saw a growing need in our community, and we believed that more people deserve the same high-quality, team-based, patient-centred care that Queen Square patients have been receiving.
We want to reassure our existing patients that this expansion will not negatively impact your care! We are committed to maintaining access, continuity, and quality for the patients and families who already trust us with their care.
We are proud and honoured that our proposal was approved by the government. This approval reflects confidence in our model of care and our ability to responsibly extend it to more patients and communities.
As we grow, we plan to go beyond our funded targets by using collaborative, innovative, and team-based approaches that strengthen care coordination, accessibility, and health outcomes.
Importantly, we are also establishing Queen Square Voices. We invite patients and family members to partner with us by sharing their voices, experiences, and ideas to help shape how care is delivered as we grow.
