What are Pharmacist Services?
Our Registered Pharmacists work within our collaborative team environment to support patients, clinicians, and staff across a variety of different programs. Their expertise ensures the safe and effective use of medications to help you achieve your health goals.
Who is this for?
These services are for rostered patients of Queen Square Doctors who have questions about their medications, are looking for support to quit smoking, or are part of other clinical programs at our clinic.
How to Access These Services
To access one-on-one services with our pharmacists, a referral from your Queen Square family doctor is required. Please speak with your physician to discuss your needs.
What services are included?
Our pharmacist services include, but are not limited to:
Medication Assessment & Review
Our pharmacists will look at each medication you are taking, counsel you on what it is for, how to take it, and what to monitor for. This can also include checking for drug interactions and identifying the cause of potential side effects.
Smoking Cessation
A one-on-one consultation service that offers helpful strategies and information to help you quit or reduce smoking. This can also include access to smoking cessation aids and nicotine replacement products.
Group Education
Our pharmacists contribute to education workshop on topics such as chronic pain management, hormone therapy for gender affirming care and supporting better sleep.
Questions to Ask About Your Medications
To get the most out of a discussion with your pharmacist or doctor, consider these 5 questions:
Changes?
Have any medications been added, stopped, or changed, and why?
Continue?
What medications do I need to keep taking, and why?
Proper Use?
How do I take my medications, and for how long?
Monitor?
How will I know if my medication is working, and what side effects do I watch for?
Follow-up?
Do I need to see my doctor for any monitoring or tests, and when?
