Overview

At Queen Square Family Health Team, providing a safe and affirming space for transgender and non-binary clients is a priority. Our team-based program offers comprehensive support throughout your transition journey, from education to medical therapies.

Please note: To best manage intake, new patients seeking to start medical gender-affirming care are currently being directed to our partners at the Prime Care FHT. Full details on the process for all patients are outlined below.

At Queen Square Family Health Team (QSFHT), transgender and non-binary clients are a priority. We are committed to providing safe, supportive, and inclusive health and counselling services that affirm your gender identity and lived experience. Our care is delivered through a collaborative, team-based approach led by a nurse practitioner, with additional support from our nursing, mental health, and pharmacy teams.

At this time, new clients seeking gender-affirming care are being directed to our partner clinic, Prime Care Family Health Team Gender Care Clinic in Milton. They will be able to support you with your gender-affirming care needs.

How Existing Patients Can Access This Program

Existing rostered patients who are interested in our gender-affirming care services should begin by speaking with their family doctor. They can provide more information and initiate a referral to our team.




Services for Existing Patients

Our team works together to ensure you receive comprehensive and personalized care. Services include:


  • Education and information about gender-affirming care.

  • Support for social, medical, and legal aspects of transition.

  • Surgical assessments and referrals.

  • Sexual health education and screening.

  • Medically supervised hormone therapy.

  • Psychosocial support and one-on-one counselling throughout your journey.




Educational Workshops & Community Groups


In addition to our clinical services, we offer the following opportunities, which are open to the entire community.


Navigating Hormone Therapy Workshop (Open to the Community)

As part of our educational offerings, we host a free, 60-minute virtual workshop focused on understanding hormone therapy. The session includes a 45-minute presentation covering medications, risks/benefits, and managing side effects, followed by a 15-minute Q&A.



CBT Affirm Support Group (in partnership with Pflag and AYSP)

We offer CBT Affirm, an 8-week support group for LGBTQ+ individuals (ages 15-29). This group is open to the community, and you don't need to be a patient of the clinic to join. It is a safe space to build coping skills and connect with peers.
