
Seniors' Wellness Program
Overview
Our goal is to help you age with confidence, dignity, and the care you deserve. The Seniors Wellness Program provides collaborative, team-based support for older adults (65+) facing various health challenges. Our multi-disciplinary team of nurse practitioners, occupational therapists, pharmacists, and physicians works with you and your family doctor to assess risk factors, address barriers to aging well, and create a personalized care plan to help you stay well and independent.

About
What is this program about?
Our Seniors Wellness Program is designed to support healthy aging for adults 65 years and older. Our goal is to help our seniors maintain their health, safety, and quality of life by providing comprehensive assessments and personalized support from our dedicated, multi-disciplinary team of healthcare professionals.
Who is this for?
This program is for older adults (65+) who are patients of Queen Square Doctors and are experiencing medical, psychosocial, functional, cognitive, and/or physical frailties.
How do patients access this program?
This program is available to rostered patients of Queen Square Doctors. To access this service, please speak with your family doctor to determine if it is right for you and to request a referral.
What services are included?
Depending on your individual healthcare needs, you may be connected with one or more members of our team for personalized support, including:
Medication review with our pharmacist.
Fall prevention and home safety guidance from our occupational therapist.
Assessment of memory and cognitive function.
Management of common age-related symptoms.
Support for caregivers navigating their roles.
Connections to community resources to help you stay well and independent.
What can patients expect?
You will be scheduled for appointments with different members of our healthcare team, with each appointment lasting between 60 to 90 minutes on separate days. During these visits, our team will assess any risk factors that may impact your health and well-being and will work closely with you and your family to create a plan that addresses any barriers to aging well.
Preparing for Your Appointment
To help us provide the best care, please bring the following to your appointment (if applicable):
Glasses and/or hearing aids.
All of your current medications, including over-the-counter items (e.g., TylenolⓇ), vitamins, and supplements.
A family member or friend who can help share information about your health.

Join Our Senior's Social Club
Our Seniors' Social Club is a monthly educational and social gathering designed for adults aged 60 and older. Each two-hour session helps you learn about healthy aging, interact socially, and enjoy brain-engaging activities. Hosted by our Occupational Therapists, the first hour features a talk by a guest speaker, followed by an hour of social chit-chat. This club is a welcoming space for seniors, caregivers, family, and friends to connect and learn together.
