
Diabetes Care Program
Overview
The Queen Square Diabetes Care Program empowers patients (18+) to prevent, manage, or delay pre-diabetes and Type II Diabetes. In partnership with your family physician, our dedicated team of specialists will help you set and achieve personalized blood sugar and health targets. We support you at your own pace with a comprehensive plan that includes group education, one-on-one appointments, and medication management. A physician referral is required to join.

About
What is this program about?
The Diabetes Care Program is offered to help you prevent, manage, and/or delay pre-diabetes and Type II Diabetes, along with related conditions such as heart disease or stroke. You will work with our program team and your family physician to set and work towards personal health targets. Once you meet these goals, you will transition back to your family physician for ongoing diabetes care.
Who is this for?
This program is for patients (18+), who are rostered with a family physician at Queen Square Doctors and have an identified need for diabetes management.
How do patients access this program?
Talk to your family doctor if you are interested in being referred to this program. They will be able to assess your needs and initiate the referral process.
What services are included?
Depending on your care needs, you will have access to our collaborative team of healthcare providers:
Registered Dietitian (RD)
Provides personalized Medical Nutrition Therapy (MNT) to help you make tailored dietary changes that align with your lifestyle, empowering you to reach your blood sugar goals.
Registered Pharmacist
Conducts thorough medication reviews to ensure the safe and effective use of all your prescribed medications and prevent potential drug interactions.
Mental Health Support
Offers 1-on-1 counselling for mental health challenges that may be impacting your diabetes management.
What can patients expect?
Upon referral, you can expect to attend a series of appointments tailored to your needs:
Diabetes Care Group Education Session
A foundational session providing an overview of diabetes management.
Initial and Follow-up Appointments with the Registered Dietitian
Crucial one-on-one sessions are dedicated to creating and monitoring your personalized lifestyle changes.
Regular Diabetes Visits with Your Family Doctor
Ensures continuity of care and integration of your program progress into your broader health plan.
As required
Medication reviews with a Pharmacist, and referrals for retinal screening or chiropody.
What happens after the program?
Once your blood sugar target is achieved, your family doctor will continue to oversee your diabetes care. You will receive a personalized care plan outlining essential yearly activities to help you continue to live well with or prevent diabetes.

Educational Workshops
Group education is a key component of our diabetes care. We offer specialized workshops for both Pre-Diabetes and Managing Type 2 Diabetes, available to patients referred by their Queen Square physician. These sessions are facilitated by our Registered Dietitians and provide practical, evidence-based strategies to support long-term diabetes management.
In addition to our referral-based sessions, we also offer a range of free community workshops on topics such as healthy eating and stress management. These sessions are open to everyone—no referral required—and are a great starting point for building skills to support your overall health and well-being.
