Chronic Pain Management Program

Overview

Living with chronic pain can be challenging, but you don't have to manage it alone. Our Chronic Pain Management Program provides a supportive, team-based approach to care. Working with our Occupational Therapist and Pharmacist, you will develop a personalized action plan that incorporates strategies for movement, medication management, and mindfulness. Our goal is to empower you with the tools and confidence to manage your pain effectively and regain control of your daily life.

What is this program about?


This program is designed to provide patients with a multi-faceted approach to managing chronic pain. Instead of focusing on a single solution, we use a combination of education, physical strategies, and medication management to create a comprehensive care plan tailored to your specific needs and goals.




Who is this for?


This program is for rostered QSD patients (18+) who have been living with non-cancer-related chronic pain lasting for more than 3-6 months and are looking for strategies to improve their quality of life.




How do patients access this program?


We accept referrals to the Chronic Pain Management Program from your Queen Square family doctor, health care provider, or self-referral. Please speak with your physician or healthcare provider to see if this program is right for you. You may also call us at 905 453 1453 x1 for more information.




What services are included?


Our program is built on the "3 M" principles: Movement, Medications, and Mindfulness. Your personalized care plan may include:


  • One-on-one consultations with an Occupational Therapist to work on movement, function, and goal-setting.

  • Medication reviews with a clinical Pharmacist to ensure your medications are safe and effective for your pain management.

  • Education on how pain affects the brain and nervous system.

  • Introduction to the Chronic Pain Workshop as a foundational step in your care journey.




What can patients expect?


After a referral from your doctor, patients may be enrolled in a 90-minute group workshop to build foundational knowledge about chronic pain management. From there, follow-up appointments and other one-on-one resources will be organized based on your individual needs and interests.


Note: one-on-one services with the Occupational Therapist and Pharmacist are only available for rostered patients.
